Delivery note sign for boxes missing?

I notice that the delivery note now doesn’t have a box on the bottom for signing, date, print name.
I realise that this can be added to the new ‘themes’ but I don’t know the html text to add.
Is it possible for these boxes to be added as standard default or can I add Html to the basic theme?
