the csv file should be in the format with correct column headings as explained in the guide.

Does your bank support any other format other than csv? Other formats are better defined so more reliable.
Can you use another date format, the one used is ambiguous for a single date
When you look at the file in a text editor (not spread sheet program), is it in csv format?
