Goto A5 Paper Size Printing from email settings - #35 by Previsam now and copy re-install the theme. It should work now.

Replace {{ business.name }} with {{ business.logo }}. Note that this will remove the business name from the template and show the logo instead.

Sorry that was a mistake you should replace {{ business.name }} with
{% if business.logo != null %}<p style="text-align: left; width: 300px"><img src="{{ business.logo }}" style="max-height: 150px; max-width: 300px" /></p>{% endif %}

If you want the logo centered just change left to centred.
