No data displayed on Delivery Notes form

I am sure this is a bug, as this is the first i have experienced it. Description UOM, Qty all columns are not being displayed in our progeam. I have checked by changing all themes and using defualt ones.

Only inventory items are being displayed and if i remove the inventory item the lines, the data everything gets wiped out…

Help ?

Delivery Notes are to record the delivery of items from inventory not of services

So no inventory item, no data

May as well be, It does not allow bugs to be ignored however
