What are you trying to add these custom fields to? Have you read the Guide: Use custom fields | Manager
like @tut said it can be achieved with custom fields.
but @john1793 if your question was about positioning these custom fields beside the document details, this can be achieved only by a custom theme. if you are familiar with programming you can easily do it yourself or you can hire someone locally to do it for you.
