Delivery Notes

Can anyone help, I have created 3 fields for my delivey note

Received By
Print Name
Date Received

I have ticked the box to show on Form, the fields are present on creation and edit but if I print the delivery note all 3 fields are missing. I am using the Cloud Version of Manager which I think is excellent.

Also, is it possible to print a delivery note with prices on it.

I have found the item that shows put something in the fields that I create but there are no boxes around the fields
