Improved in the latest version (22.10.21).
When entering exchange rate, you can specify whether the rate is Base Rate
or Counter Rate
And then this rate is shown on the list of exchange rates.

How can we set individual transaction conversation rate?
You cannot. What you can do is force equivalence when paying or receiving funds against foreign currency invoices so they remain paid in full when exchange rates fluctuate in the future.
