T4 slip Canada payroll

Hi all,
I m doing my assignment regarding T4 slip (Canada) hv few doubt someone please help me

For employees who

  1. submit a completed cpt30 form during the year, electing to stop contributing to cpp
  2. submit a completed cpt30 form during the year, revoking their previous election to stop contributing to cpp

For this two employees what to enter on T4 slips in box 16,26,28

Please help me
