When using the Print button from Manager the report prints fine.
When using the PDF button from Manager some of the boxes are too narrow and unusable.

Three fields are affected being 2 x Type boxes and Signature of authorized person box.
The latest version of the report puts default values into those boxes to maintain the height. Download it from https://www.manager.io/localizations/au
As for the signature, if you upgrade to the latest version of Manager too, it will show as a line. I think the box would be too small for a signature anyway.
